Product Backlog refinement (also called grooming) is a shared responsibility but primarily led by the Product Owner. When a team repeatedly fails iterations due to ambiguity(unclear requirements), the Project Manager should guide the Product Owner to improve product backlog refinement processes. This addresses the root cause collaboratively rather than changing iteration length, limiting WIP, or focusing on individual performance.
In my opinion B is the right answer, since the requirements are unclear, I do feel the product manager needs to go in to review the refinement process to ensure clarity on all assigned requirements
Here the keyword is unclear requirement D is a not a first step when whole team is not able to understand the requirement. A, B are useful when there is a problem of overwhelming work Only C is the correct option, Backlog refinement helps clarify requirements by involving the product owner, stakeholders, and development team.
